Community Clinic of High Point

Electronic Medical Record System

Provides capacity building support to help implement an electronic medical records (EMR) system. Implementing an EMR will help the Clinic to have improved coordination of care with specialty/acute care providers and will streamline external communications with medical partners and pharmacies. Funding will support staff training, hardware/IT support, and digitizing all active charts.

Community Housing Solutions of Guilford, Inc.

Aging Gracefully in Place Program

Supports expansion of the Aging Gracefully in Place home program to help with home repairs, accessibility modifications, and durable medical equipment and assistive equipment for High Point homeowners over the age of 65, allowing them to live longer in their homes. The program was developed in partnership with researchers at Johns Hopkins University to increase older adults’ mobility, functionality, and quality of life.

D-UP BASKETBALL FUNDAMENTALS AND SKILLS TRAINING

Capacity-Building Expansion Effort

Funding provides capacity-building support for significant expansion in the Washington Street neighborhood, including two new staff positions enabling the organization to focus on community development and strategic initiatives. D-UP provides various wraparound services and programs to support youth's health, education, and life readiness.

Family Service of the Piedmont

Child First Program

Provides startup funding to implement the Child First program. This evidence-based, intensive early childhood intervention works with the most vulnerable young children (prenatal through age five) and their families. The program includes various services, including psychiatric evaluation, medication management, child and family therapies, and coordination of care and case management.

Foundation for Health Leadership and Innovation

Healthy Minds Initiative Strategic Partner

Foundation for Health Leadership and Innovation's Center of Excellence for Integrated Care (COE) serves as the Foundation’s strategic partner and project manager for the Healthy Minds initiative by providing technical expertise for implementing evidence-based integrated care systems, accountability and oversight, and opportunities for collective advocacy and collaboration. Funding continues to support staff time and program-related expenses.

Greater High Point Food Alliance

Healthy Guilford Coalition Planning Efforts

Greater High Point Food Alliance plans to increase its administrative capacity and position itself for growth and potential expansion. Funding supports a portion of costs for an administrative position and will enable the organization to lead planning efforts in The Duke Endowment’s Healthy People, Healthy Carolinas initiative.

Greensboro Housing Coalition will explore the opportunity to expand its scope of programs and services to High Point. Funding will support the organization to convene stakeholders to better understand the capacity and scope of services needed in High Point, conduct a feasibility study, and develop an organizational plan for expansion.

Program Expansion & Capacity Building

Funding supports the organization in hiring a new part-time coordinator position and project expenses related to expanding educational programming that will increase the number of people receiving or purchasing fresh, healthy food in under-resourced neighborhoods, increase the number of cultivated acres and farm sales, and increase existing farmer productivity.

Guilford Adult Health Inc.

Increased Access to Integrated Behavioral Health Services

The grant provides continued funding for the staff salary for a Behavioral Health Clinician who is a licensed clinical social worker (LCSW) with medical specialization to provide integrated behavioral health services at High Point Medical Center and the Community Clinic of High Point. Services include immediate on-site counseling, coordination of care with patients’ primary care providers, and referrals to community resources. The Clinician educates patients on stress management and ineffective patterns of healthcare utilization and provides behavioral health and/or substance use diagnoses.
